Comments:
An unannounced monitoring inspection was conducted on March 12, 2024, from 10:00 AM until 11:35 AM. There were 21 children present, ranging in ages from 12 months to 5 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of five (5) child records, the center did not ensure that all records were complete. Evidence: 1. The record for child #2 (enrollment date 8/1/2023), child #3 (enrollment date 5/23/2022), and child #5 (enrollment date 3/6/2023) did not contain the parent?s work number. 2. The record for child #3 did not contain documentation of child updates and confirmation of up-to-date information the child?s record.

Will make sure all files have a work number. Will get parent to update.

Description:
Based on observation, the center failed to ensure that all equipment shall be maintained in a safe and operable condition. Evidence: 1. On the preschool playground there was two tricycles that had the handlebar grips missing exposing the metal ends. 2. On the preschool playground there was a dump trunk and two tricycles that were rusted.

Description:
Based on observation, the center failed to ensure that children under 10 years of age are always within actual sight and sound supervision. Evidence: 1. During the inspection, a 3-year-old walked out of the Bumble Bee classroom to go to the restroom across the hall. Staff #1 left 6 children between the age of 4 years ? 5years alone while attending to the child in the restroom.

Will make sure that teacher is in sight and sound at all times
