Comments:
An announced initial inspection was initiated on 12/20/2021 and concluded on 12/20/2021. The director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, special care and emergencies. Areas of the facility, both inside and outside, were inspected.
Information gathered during the inspection determined a non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.
The facility was reminded that the initial application is not determined to be complete until all required background check documents and certificates are received for the owners/officers/agents of the limited liability corporation.
Based upon observation, the facility has not ensured that all areas of the center, including the outside are maintained in a safe condition. Evidence: The area surrounding the base of four fence supports has been eroded away exposing the concrete footings.
Plan of Correction:
The facility responded with the following: The areas around the footings of the fence posts will be filled with soil to cover the exposed concrete. The area will be monitored to keep the footings covered until the new fence is installed within the next six months.
