Comments:
An unannounced monitoring inspection was started in the center on September 13, 2023 from 11:45 AM until 12:45 PM and concluded on September 14, 2023. At the time of entrance, 46 children were in care with 6 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during preparing for and eating lunch.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on documentation review and staff interviews, the licensee did not ensure to obtain an annual approval from the health department. Evidence: Center Director confirmed the most recent copy of the health inspection available for review was dated 11-29-2021.
