Comments:
An announced initial family day home inspection was conducted on Tuesday, February 22, 2022. A complete walk through inspection of the home and outdoor areas was conducted with the provider. The areas of the home that will be designated for child care and required documentation was reviewed. The provider has submitted all of the required application materials at this time. Though, some standards of care could not be determined during the initial study, the provider is in substantial compliance with the standards that could be determined. A recommendation for a six-month conditional license will be made at this time. During the conditional period, the provider will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure.

Based on observation and interview, the provider has not ensured that children under two years of age do not have access to stairs.
Evidence: 1) During the inspection, the bottom of the stairs leading to the second story of the home did not have a gate to prevent access by children. The applicant will serve children ages 2 months to 12 years old. 2) During interview, the provider reported the gate has been ordered, but not yet installed.
Plan of Correction:
The gate was installed on 02/22/2022 and proof was submitted to the department via email.
