Comments:
A renewal inspection was initiated on October 5, 2022, the licensing inspector was on site from 11:20 AM to 12:30 PM. There were no children enrolled in the center at the time of inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. The provider stated that transportation is provided and the vehicle and transportation documents were reviewed. Fire Inspection, health inspection and certificate of liability were reviewed.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Based on a review of the center's injury reports, all required information was not documented.
Evidence: Injury reports for the month of June and July were reviewed; Five injury reports did not have any future action to prevent recurrence of the injury, two injury reports did not have time the parent was notified, and two injury reports did not have staff and parent signatures or two staff signatures.
