Comments:
An unannounced renewal inspection was conducted on August 9, 2023 from approximately 9:50 am to 11:00 am. Provider was present with four children. Children were observed participating in various floor activities. Provider and four child records were reviewed. Violations are noted on the Violation Notice.
Based on review, provider did not ensure 2 of 4 child records have documentation of required information.
Evidence: The following was found during review of children's records: 1. The record for child #1 (start date 6/6/2023) did not have a physical examination. 2. The record for child #4 (start date 5/3/2021) did not have parents' employment phone numbers.
Plan of Correction:
Provider will get missing information from parents.
