Technical Assistance:
The provider has been instructed to submit an initial application to transfer her old license to her new address. The provider has been instructed to submit the proper zoning form for the new address. The provider has been instructed that an Arlington County license is required to operate a child care in Arlington County.
Comments:
An announced initial inspection was conducted June 20 2024 between 1:00pm and 2:00pm. There are currently no children enrolled in the home day care. The provider is licensed at a previous address, but is not currently license at the new address until all required documents have been received, reviewed, and approved. Based on record review, the provider did not ensure that the emergency plan is updated as needed.
Evidence: The emergency preparedness plan has not been updated for the new home. Items in the plan still reference the previous address and evacuation locations for that space. New information is needed for the new home.

Description:
Based on observation, the provider did not ensure that the home is maintained in a safe condition.
Evidence: While walking through the home, one of the stairs had a piece move away from the stair. The provider proceeded to place it back, but it was easily moved again.
