Comments:
Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on February 1, 2024. There were 2 children ages 4-6 years old present under the direct care of 3 staff. The Licensing Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on inspection, the Center failed to ensure each staff person completes an annual health exams for each staff person.
EVIDENCE: Current annual health exams were unavailable for Staff #2 and Staff #3. Health exam for Staff #2 was last completed 3/30/2022. Health exam for Staff #3 was last completed 3/30/2022.
