8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes
Comments:
A subsidy health and safety (SHSI) inspection was initiated on 9/20/2023 from 9:45 am- 11:315am. There were 2 children present with 1 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of attendance records and provider interview, the vendor has not maintained daily attendance that documents the arrival and departure of each child in care. Evidence: The vendor is not maintaining attendance when the child arrives and departs. There were 3 children that according to the vendor, had come in the morning but had left by the time we arrived at 9:45 am. The 3 children were still marked present on the attendance log. The attendance log was also prepopulated for the 21st and 22nd with the 5 children names already written on the attendance log.
Plan of Correction:
Attendance will be taken as the children arrive and marked when they depart.

Based on caregiver interview, the caregiver has not completed the annual health and safety update course. Evidence: The vendor stated that she has not taken the annual health and safety update course.

I will take the annual training and keep the certificate in my record.
