Comments:
An unannounced renewal inspection was initiated and concluded on Monday, March 18, 2024, to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from 10:15am to approximately 12:05pm. There were four nonresident children present with one provider. The inspector reviewed compliance in the areas of administration, physical plant, and programming. A total of one caregiver's records, one household member's' records, and one household pet records were reviewed.
Information gathered during the inspection determines non-compliance with applicable regulations or law. Violations were documented on the violation notice issued to the program.
Based on a review of four children's records, the provider did not maintain the required information for two children in care.
Evidence: The records of Child #3 (First Date of Attendance: 10/17/23) and Child #4 (First Date of Attendance: 08/21/23) did not contain the address of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care.
Plan of Correction:
The missing information will be requested from the parents today.

Description:
Based on record review and observations, the provider did not ensure to meet the criteria on the health and safety checklist at the time of the monitoring visit.
Evidence: 1) The record of Child #4 (First Date of Attendance: 08/21/23) did not contain documentation of a physical examination.
2) The evacuation drills reviewed did not show documentation that the provider has conducted emergency evacuation drills monthly. The last documented emergency evacuation drill was on November 17, 2023.
3) The tuberculosis (TB) screening in the record of Household Member #1 expired on 02/18/24.
Plan of Correction:
Moving forward, the drills will be documented when practiced and the physical examination will be requested from the parent today. Household Member #1 will schedule an appointment to obtain an updated TB.
