An announced initial inspection was conducted on 3/24/2022. No children were in care. The inspection included a tour of the home and outside play area and review of the health and safety checklist.
Information gathered during the inspection determined non-compliance with applicable requirements and violations were documented on the violation notice issued to the program.
Based upon review of documents provided, the applicant has not obtained a tuberculosis screening within 90 days of the date of the initial application Evidence: The tuberculosis screening on the document provided by the applicant is dated 07/21/2021, which is not within 90 days of the date of application (application received 2/17/2022).

Description:
Based upon observation, the provider has not ensured that all of the criteria on the health and safety checklist have been completed. Evidence: 1. There were knives and other sharp utensils in two cabinet drawers in the kitchen. 2. There was peroxide out on the sink and personal hygiene products with warning labels in the medicine cabinet on the wall in the bathroom. 3. There were four trash bins overflowing with trash in the back play area. 4. There was a trampoline in the back play area. 5. There was a white metal crib without documentation that it met the current Consumer Product Safety Commission (CPSC) guidelines for cribs are used for children under 18 months of age.
