Comments:
An initial inspection was conducted today between the hours of 9:30am-11:30am. No children are currently in care and two staff members were on site. Space calculations for eight classrooms and the playground was conducted. The classrooms were set up with adequate programming equipment, materials, and space to store each child's belongings. The playground was observed having adequate resilient surfacing and equipment was determined age appropriate for the requested age range. Cribs were observed on site for infants. Two staff files were reviewed. Areas of non-compliance are identified in the violation notice. Thank you for your cooperation today. Violations:
Standard #: 22.1-289.058
Description: Based on observation and interview, the center was not equipped with at least one carbon monoxide detector.
Evidence:
1. The building that was built before 2015 that houses a child day program was not equipped with at least one carbon monoxide detector.
2. Staff #1 confirmed that the center did not have a carbon monoxide detector on site.

Plan of Correction: A carbon monoxide detector will be purchased and placed in the building.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. A corner post of the playground wooden fence located near the roadside had deteriorating and splintering wood along the edges of the post.
2. Seven loose and warped boards were protruding out alongside the right side of the playground fence located near the building. Two of the boards contained two or more rusty nails protruding outwards.
3. A four inch nail was protruding out of the top of a wooden post located near the picnic table on the playground.

Plan of Correction: A pre existing work order has been placed to address the fence issues and the violation items will be added to the work order.
