Comments:
A self reported incident was received by the licensing office on Monday, June 24, 2024 regarding allegations in the areas of Forbidden Actions.
A joint investigation with two local government agencies was initiated and concluded on July 2, 2024.
The Center's video recordings were observed and staff and children records were reviewed in response to the allegations.
The evidence gathered during the investigation support ed non-compliance with standards or law; therefore, the allegation is valid.

Violations:
Standard #: 8VAC20-780-410-1
Description: Based on video observation, staff used the forbidden action of roughly handling children in care.
Evidence:
1. In the Infant classroom on June 20, 2024 at approximately 2:38pm, Staff #1 is observed standing in front of two highchairs with Child #1(10 months old) at their feet. Staff #1 grabs Child #1's right forearm and carries them approximately 3 feet and places them on the carpet. Child #1 is not crying. Child #1 crawls towards Staff #1 by the two highchairs in the classroom.

Plan of Correction: The staff member was removed from the classroom after being informed of the incident. The center conducted a training for all staff to review the proper way to handle children to include correct handling techniques and procedures. The handling techniques and procedures were demonstrated for staff members.
