Comments:
An unannounced renewal inspection was conducted on March 14, 2023 between 10:30am and 11:30am. There are 2 children enrolled in the direct care of 1 provider. Child records, provider records, household records, pet records, the physical space, emergency drills, attendance records, the flashlight and emergency radio were reviewed and inspected. Based on interview of the provider, the provider failed to ensure compliance with the terms of the current license issued by the department.
Evidence: The current license had a minimum age of 16 months and the provider had a child enrolled that is 8 months old.
Plan of Correction:
I will request a younger age for the license.

Evidence: Child #2 record states that the child has an allergy, but there is not instructions for what action to take in case of emergency.
