The Licensing Inspector conducted an unannounced Subsidy Health and Safety inspection on Thursday, September 15, 2022 from approximately 10:00am to 11:30am. There zero children and one staff member present. A total of five children?s files and five staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of five staff records and interview, the vendor did not obtain, prior to employment, satisfactory results of a fingerprint-based national criminal background check for one staff member.
Evidence: 1. The record for Staff #1 (employment date 09/06/2022) did not contain the results of the fingerprint-based national criminal background check. 2. Staff #1 was identified as a staff member working with children during the inspection. 3. Staff #5 stated she had not received the results of the fingerprint-based national criminal background check.
Plan of Correction:
This was an oversight. We will schedule an appointment today to have the fingerprint criminal record search completed.

Description:
Based on review of five staff records and interview, the vendor did not ensure at least one staff on duty had obtained instruction in performing a daily health observation of children within the last three years.
Evidence: 1. The records for Staff #1, Staff #2, Staff #3, Staff #4 and Staff #5 did not contain documentation of Daily Health Observation training. 2. Staff #5 stated staff have not taken an official training regarding daily health observation of children.
Plan of Correction:
The staff will take the online Daily Health Observation training.
