Comments:
The Licensing Inspector conducted an unannounced Subsidy Health and Safety inspection on Thursday, September 15, 2022 from approximately 10:00am to 11:30am. There zero children and one staff member present. A total of five children?s files and five staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-550-2-a
Description: Based on review of five staff records and interview, the vendor did not obtain, prior to employment, satisfactory results of a fingerprint-based national criminal background check for one staff member.

Evidence:
1. The record for Staff #1 (employment date 09/06/2022) did not contain the results of the fingerprint-based national criminal background check.
2. Staff #1 was identified as a staff member working with children during the inspection.
3. Staff #5 stated she had not received the results of the fingerprint-based national criminal background check.

Plan of Correction: This was an oversight. We will schedule an appointment today to have the fingerprint criminal record search completed.

Standard #: 8VAC20-790-600-I
Description: Based on review of five staff records and interview, the vendor did not ensure at least one staff on duty had obtained instruction in performing a daily health observation of children within the last three years.

Evidence:
1. The records for Staff #1, Staff #2, Staff #3, Staff #4 and Staff #5 did not contain documentation of Daily Health Observation training.
2. Staff #5 stated staff have not taken an official training regarding daily health observation of children.

Plan of Correction: The staff will take the online Daily Health Observation training.
