A renewal inspection was conducted on November 2, 2022. There were no children present since the program isn't currently operating. One agent's record was reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Evidence: 1. The date of the most recent central registry check for Staff #1 was 5/27/2017. 2. The date of the most recent sworn statement for Staff #1 was dated 10/31/2022. The previous sworn statement was dated 5/9/2017.
Plan of Correction:
Have agent redo CPS form and background.
