The providers home and all areas for child care were toured. Items reviewed during the inspection included; required posted information, first aid kid, emergency preparedness, nutrition, field trip requirements, one provider record, and one household members record. The outside play area was viewed and checked for hazards.

A recommendation will be made for an initial six month conditional Family Day Home license with a capacity of 5, ranging in ages from 6 months-12 years of age.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

If you have any questions or concerns please contact the Licensing Inspector at 540-430-9257.

Violations:
Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, the provider failed to ensure that a landline telephone shall be available, operable, and accessible during the family day home's hours of operation.

Evidence:
1. There was no landline telephone installed in the home at the time of the inspection.
2. The provider verified that there was not a landline currently installed in the home.

Plan of Correction: The provider just moved to the new location and is working on getting a landline installed. There are cell phones available for use during the hours of operation.
