A monitoring inspection regarding a self reported incident regarding behavior guidance was conducted October 6, 2022. The inspector reviewed video footage of the incident and conducted interviews. The preponderance of evidence determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interview and review of video footage, behavior guidance was observed not being constructive in nature, age and stage appropriate and was not intended to redirect a child to appropriate behavior. Evidence On September 31, 2022, staff #1 was observed on video footage demonstrating behavior guidance that was not constructive in nature, age and stage appropriate and was not intended to redirect to a child.
Plan of Correction:
Staff member involved was terminated. All of staff was retrained on appropriate behavior guidance.
