A complaint investigation was initiated on 05/20/2024 from a report received by the Fairfax Licensing Office on 5/15/2024 regarding staff to children ratios. The information gathered during the investigation does not support the allegation, so the complaint is determined to be ?not valid.? There are no violations resulting from the complaint, but other violations were observed during the investigation and available for review on the violation notice. Based on review of staff records, not all staff have documentation of certification in first aid, and cardiopulmonary resuscitation(CPR). Evidence: The record for staff #1, first day of employment 2/12/2024, does not contain documentation of completing an in person first aid and CPR certification class within 30 days of first day of employment.

Based on review of staff records, children whom medications have been prescribed were not always in the care of a staff member who has satisfactorily completed the Medication Authorization Training (MAT) coarse. Evidence A trained MAT staff member is not always available during the centers operating hours.
Plan of Correction:
The MAT training for multiple staff members will be completed By June 30th.
