Based on a review of five caregiver records and interview, the provider did not ensure to obtain documentation of a negative tuberculosis (TB) screening for two caregivers at the time of hire that was completed within the last 30 days as required.
Evidence: 1. The record of caregiver #2 (date of employment: 4/17/2023) contained a negative tuberculosis screening dated 9/6/2022. The record of caregiver #5 (date of employment: 1/30/23) contained a negative tuberculosis screening dated 10/19/2022. 2. The provider acknowledged that the TB screenings were not completed within the required time frame.
Plan of Correction:
provider will obtain a current TB test by 8/1
