Based on review of documentation and staff interview, the provider failed to report to the department within 24 hours the serious injury to a child while under the provider's supervision. Evidence: On 8/24/2023, Child A suffered a serious injury which required outside medical attention. The incident was not reported to the department by the provider.

Description:
Based on documentation review and staff interview, the provider failed to notify the parent immediately when the child has a serious injury that requires emergency medical treatment. Evidence: Child A sustained an injury above the eye at 11:15 a.m. on 8/24/2023. The substitute provider called and texted the parent, but could not reach her. The provider did not contact an emergency contact. The parent picked up the child at approximately 5:00 p.m. and took the child for treatment. The injury was glued together at that time.
