Based on a review of four caregiver records and interview, the provider did not ensure that two caregivers obtained a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence: 1. The record of caregiver #2 (date of employment: 9/5/2022) contained 11 of the 16 required annual training hours. The record of caregiver #4 (date of employment: 9/5/2022) contained 13.5 of the 16 required annual training hours. 2. The provider acknowledged that the two caregivers did not obtain 16 hours of annual training.
Plan of Correction:
Spreadsheet will be created to ensure that all hours are completed. Staff training sheets will be checked periodically throughout the year.

Description:
Based on observation and interview, the provider did not ensure that when activity tables are occupied by children, they shall be used in accordance with the manufacturer's instructions the safety straps shall be used and securely fastened.
Evidence: 1. Three children were observed in seats at an activity table that contained safety straps. The straps were not fastened. Caregiver #3 was supervising the children at the table and acknowledged that the straps were not fastened.
Plan of Correction:
All workers were reminded that children need to be fastened in the seats.
