Comments:
An in person monitoring inspection was conducted today. There were 3 children present (including 2 resident children). 1 child's record was reviewed. See Violation Notice for non-compliances. Children were observed resting quietly during the inspection. All previous violations have been corrected. Based on a review of records, on child record does not contain all required information. Evidence: Child 1's record does not contain documentation of the parents work address and work phone number.
