Comments:
An initial inspection was conducted with the director beginning at 9:29 AM and ending at 10:30 AM. Facility documentation was reviewed at the main CCLC campus beginning at 12:02 PM and ending at 12:31 PM. There were no children present at the center during the inspection. Four rooms were inspected for daycare use. The facility had an adequate amount of supplies and furnishings to provide care for the age range being requested. Four staff records were reviewed. The first aid kit, flashlight, and battery-operated radio were reviewed and in compliance. The most recent health inspection was dated April 28, 2022. The most recent fire inspection was April 28, 2022.

Standard #: 8VAC20-770-40-C-1-b
Description: Based on a review of documentation, the center did not ensure that all staff had a sworn statement/affirmation upon application for licensure or registration as a child day program. Evidence: Staff A and B did not have an signed sworn statement in their record.

Plan of Correction: The staff will be provided a sworn statement form to complete and the completed documentation will be placed in their records.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure that there was a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff
member's hand nor the soiled diaper touches an exterior surface of the storage
system during disposal of diapers. Evidence: There was a plastic trashcan that needed to be opened using hands to dispose of diapers in the diapering area.

Plan of Correction: The director stated that the facility will purchase an appropriate diaper disposal system that meets the standards as soon as possible.
