Comments:
An unannounced monitoring inspection was conducted with the staff on February 13, 2023 beginning at 11:53am and ending at 12:47pm. There were 4 infants in the direct care of 2 staff. Two infants were observed asleep in cribs. Two children were observed interacting with their caregivers. Seven children?s records were reviewed. Two staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The daily attendance and emergency drill log were reviewed. The most recent emergency escape drill was documented as 12/30/2022. The most recent health inspection was dated 8/18/2022. The most recent fire inspection was dated as 4/28/2022.

Based on a review of 7 children's records, the center failed to obtain documentation of immunization prior to their first day of attendance. Evidence: Child B's (start 12/9/2022) record was missing documentation of immunization.
Plan of Correction:
The center staff will request the missing documentation from the parents. Once the missing documentation is received, it will be placed in their records

Description:
Based on a review of 7 children's records, the center failed to obtain documentation of a physical examination within 30 days of attendance. Evidence: Child A (start 11/7/2022) and child B's (start 12/9/2022) records were missing documentation of a physical examination.

The center will request the missing documentation from the parents. Once received the documentation will be placed in their records.

Description:
Based on a review of 7 children's records, the center did not ensure that all of the children's records contained the required information. Evidence: Child B's, child C, and child G's records were missing documentation of viewing proof of the child's identity and age.

The center staff will request the missing documentation from the parents. Once the missing documentation is received, it will be placed in their records.

Description:
Based on observations, record review and discussion, the center staff failed to ensure that the requirements for diaper ointment or cream as stated in the standards were met. Evidence: A tube of Desitin was observed on the counter, the Desitin was not labeled with a child's name as required by the standards. The provider confirmed that the Desitin belonged to Child G.
Plan of Correction:
The staff stated that they would label the topical ointment with the child's name.
