Technical Assistance:
LI and facility staff discussed the health inspection/health permit.
Comments:
An unannounced monitoring inspection was conducted with the director on September 19, 2023 beginning at 8:48am and ending at 9:33am. There were 4 children, ages 7 through 12 months old, in the care of 2 direct care staff. The children were observed playing with soft toys, crawling, and interacting with caregivers. Two children?s records were reviewed and in compliance. Two staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. There were no medications on site to be reviewed. The daily attendance and emergency drill log were reviewed. The most recent evacuation drill was documented as 8/23/23. The most recent shelter-in-place drill was documented as 1/17/23. The most recent lockdown drill was documented as 8/24/2023. The most recent health inspection was dated August 18, 2022. The most recent fire inspection was dated April 24, 2023.

Based on observation, the facility did not ensure that all areas and equipment of the center were maintained in a safe and operable condition. Evidence: The two cabinet doors in the smaller room with the cribs do not close completely creating a pinching hazard.
Plan of Correction:
The facility will find a different locking mechanism which will allow the cabinet doors to close completely.
