Comments:
A monitoring inspection was conducted on October 25, 2023. The inspector was on site from approximately 3:30 pm until 5:25 pm. The director was not present; however, the regional director was onsite and assisted with the inspection. There were 60 children present, ranging in ages from five years to twelve years, with six staff supervising. Children were divided into groups according to their grade level. Each group was observed participating in a variety of activities including outdoor play, decorating mask, playing with games and Legos, reviewing long and short vowels and creating posters for huddle teams. later children ate a snack. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records were reviewed. Five staff records were reviewed virtually on October 26, 2023. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of ten children?s records, the center did not have a written allergy action Care plan for four children diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction.
Evidence:
1. The record for Child #7 (enrolled 10/02/2023) contained documentation of a diagnosed food allergy for the child. A written food allergy action plan had not been obtained. 2. The record for Child #8 (enrolled 10/02/2023 contained documentation of a diagnosed food allergy for the child. A written food allergy action plan had not been obtained. 3. The record for Child #9 (enrolled 10/02/2023) contained documentation of a diagnosed food allergy for the child. A written food allergy action plan had not been obtained. 4. The record for Child #10 (enrolled 10/02/2023) contained documentation of a diagnosed food allergy for the child. A written food allergy action plan had not been obtained.
