3/6/2023 Arrived 3:00pm Departed 4:15pm A subsidy health and safety (SHSI) inspection was completed. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined violations with applicable standards or law. A violation notice wasissued.
Based on a review of staff files, the facility did not ensure that all staff have completed the required 10 hour preservice training within 90 days of hire. Evidence: 1. Staff #1 has not completed the preservice training. Hire date 9-1-22
