10-11-23 Arrived 3:10pm Departed 4:30pm An unannounced inspection was conducted today. There were 19 children present with one staff. The children were doing homework and eating a snack. Two child records and two staff records were reviewed. Thank you for your cooperation. Please contact me if I may be of service.
Based on a review of staff files, the facility failed to ensure that all repeat fingerprint background checks are conducted every five years. Evidence: 1. There is no repeat fingerprint background check on file for Staff #1. The last fingerprint on file is dated 10-4-18

The program director reported that a repeat background check will be conducted.

Description:
Based on observation, the facility failed to ensure that one staff member and an immediately available staff member, volunteer or other employee who is at least 16 years of age are present when children are in care. Evidence: 1. At 3:25pm there were 19 children present with one staff member. A second staff member arrived at 3:40pm.
