Comments:
An unannounced renewal inspection was conducted on 2/8/2023 for the Y After School at Grandin Court Elementary. There were 17 children, ages 5 -10 years and 2 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 1 staff record were reviewed. The children were observed during homework time and active time on the playground. The inspector arrived for the inspection at 3:50pm and departed the center at 4:48pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
The inspection findings have been updated as a result of a review completed by the Licensing Administrator on 3/1/2023
Based on the review of three children?s records, the center failed to ensure that the separate record for each child enrolled contains all of the elements as required by the standards.

Based on observation and discussion with staff, the center failed to implement a monthly practice evacuation drill.
Evidence: Documentation of monthly evacuation drills was unavailable and on the day of the inspection there have been no emergency evacuation practice drills completed during the first six months of the school year.

Provider will document emergency evacuation drills when completed.
