Comments:
An unannounced complaint inspection was conducted today in response to a complaint received about inadequate food portions being served to students during meals and snacks. Interviews were conducted, menus reviewed, 2 staff record, and 1 child record were reviewed. Interviews conducted during the investigation found a preponderance of evidence to support the claims made in the complaint, and it is deemed valid. One additional non complaint related violation was cited during today's inspection.

Description: Based on interviews, children under the age of 10 years of age were not always within actual sight and sound supervision of staff.
Evidence:
On April 12, 2024, Staff 1 observed Child 1 alone and unattended on the preschool playground. Child 1 has been out of sight and sound supervision for an indeterminate period of time, but interviews conducted suggest the child was alone for approximately 5 to 8 minutes.

Based on interviews, the center failed to follow the most recent, age appropriate nutritional requirements of the Child and Adult Care Food Program (CACFP) of the United States Department of Agriculture (USDA), or provide extra servings of food when requested.
Evidence:
1. Multiple staff interviews confirmed that the serving size of bananas served at morning snack was not in line with CACFP meal pattern guidelines. On recent occasions 1/4 of a banana was offered as a snack component to children regardless of age.
2. Multiple staff interviews confirmed that toddlers were offered a protein serving of one small meatball for lunch. Extra meatballs were not available for the children before the class had begun napping.
3. Multiple staff interviews and photographs taken by staff, showed that a recent lunch had a vegetable serving consisting of one piece of lettuce.
