On 11/13/2023, the department received a complaint regarding an injury to a child at the center. On 11/17/2023 the licensing inspector initiated an inspection along with a representative from a local agency at the center. Interviews were conducted, documentation reviewed, and observations were made in the center. On-site inspections were conducted on 11/17/2023 and 11/27/2023. Additional documentation was reviewed off-site. The inspection was completed 02/27/2023. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.
Based on interview and review of a report of child injuries, the provider failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. Evidence: The department was notified by a local agency representative on 11/13/2023 of a child injury that occurred on 11/09/2023 while the child was under the supervision of the center and the child was taken to the hospital by a parent. The department received the required notification report from the center on 11/27/2023.

8VAC20-780-340-A Complaint Related Based on interviews and written documentation, the provider failed to ensure a child?s care, protection, and guidance.
Evidence: The licensing office received notification of a child injury that occurred at the center on 11/09/2023. Staff, S2 stated that child, C1 (age 2) woke up from a nap with a large red mark on his cheek that looked like a bite mark. The incident report completed by S2 dated 11/09/2023 at 2:56 p.m. documented ?I didn?t see anything when I changed before nap, but when he woke up, he had a large red mark on his check that looks like a bite mark.? S2 reported sending a picture of the child?s face with the notification to the parent of the injury through the center?s communication application. Staff, S1reported receiving an email from the parent of C1 on 11/13/2023; the parent reported the child had been taken to the hospital for treatment after the incident and the medical report identified the injury as a bite mark.
Incident reports document child, C2 (age 2) biting other children on thirteen occasions during the months of September, October, and November 2023. Incident reports document child, C3 (age 2) biting other children on three occasions in October and November 2023.
