Based on record review, and interview, the vendor failed to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for child #1, date of enrollment 8/21/23, revealed a food allergy. The record did not contain a written care plan for child #1 to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Interviews with the vendor confirmed that the record did not contain a written care plan for child #1.
Plan of Correction:
I will make sure each child with a food allergy listed on their registration form has a written care plan from their physician before starting.
