A non-mandated inspection, resulting from a self-reported incident that occurred on 9/14/2023, was conducted by licensing staff on 9/18/2023. The inspector interviewed 2 staff.
Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on interviews, the center failed to ensure that all children under 10 years of age shall be within actual sight and sound supervision of staff.
Evidence: Evidence: 1. Staff #2 stated that the parent of Child #1, age 5, called at approximately 4:10PM and let Staff #2 know that they were arriving to pick up Child #1. Staff #2 walked to the opposite end of the gym to get the sign out clipboard and did not see Child #1 leave the gymnasium. Staff #2 did not know Child #1 was gone until Staff #1 let them know that Child #1 had left the gym and exited the school across the hall from the gym. Staff #2 exited the school and Child #1 was standing at the curb approximately 25 feet from the school exit as their parent pulled up. Staff #2 stated that Child #1 was out of sight and sound supervision for approximately 1 minute.
2. Staff #1 stated they were in the hallway with another child in the restroom when they saw Child #1 exit the school across the hall from the gym. Staff #1 stated they informed Staff #2 that Child #1 had exited the school building.
Plan of Correction:
Provider will ensure staff maintain sign and sound supervision of children at all times.
