Comments:
An unannounced monitoring inspection was conducted on 05/06/2024 and concluded with an electronic record review on 05/09/2024. There were 22 children, ages 3 years - 5 years, 4 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during teacher directed activities, child directed activities and outdoor time. The inspector discussed the following with the provider: children?s records. A total of 5 children?s records, and 5 staff records were reviewed. The inspector arrived for the inspection at approximately 9:15 am and departed at approximately 10:15 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review and interviews with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.
Evidence: The record for Staff 2 documented a hire date of 04/15/2024. The record for Staff 2 did not contain the following: documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff 5 and 6 confirmed this information was not available.
Plan of Correction:
HR will follow up with employee to obtain references.
