An unannounced monitoring inspection was conducted 2/21/2024 from 10:05am through 1:15pm to investigate a self-reported incident received by the Northern Licensing Office regarding staff engaging in a forbidden action. Staff and child records were reviewed, staff interviews were conducted and the center's video recording was reviewed. A violation was found regarding a forbidden action and is identified in the Violation Notice.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 4/8/2024. Please email me at morgan.bryson@doe.virginia.gov with any questions.
Based on review of the center's own video recording, a staff member engaged in a forbidden action of striking a child.
Evidence: On 2/7/2024 at approximately 9:34am Staff #1 quickly approached Child #1 after the child threw a book down, bent down in the child's face with an angry expression, and jabbed Child #1 in the left side rib area.
