Comments:
Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on October 5, 2023. There were 8 children ages 2 1/2 to 4 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 2 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observations, the Center failed to post the fact that the Center is exempt from licensure in a visible location on the premises.
EVIDENCE: Exemption letter was not posted.
Plan of Correction:
Director will post the exemption letter on the parent information board.

Description:
Based on discussion with staff, the Center failed to have a least 1 staff person present during the center's hours of operation who has received training in the daily health observations of children.
EVIDENCE: There was not at least one staff person present who has received Daily Health Observation training.

Description:
Based on record review, the Center failed to have required information from the person enrolling the child regarding previous child day care and schools attended by the child.
EVIDENCE: Records of Child #1 and Child #2 did not include information regarding previous schools/daycare attended by the child.

Will request information from the parents. Documentation will be maintained in the child's record.
