Comments:
An unannounced non-mandated inspection was conducted today. A total of 6 children (13pts) with the Provider were present. At the beginning of the inspection, the children were observed playing with a new children's work bench equipped with interactive toys such as drills, wood, helmets and safety goggles. All of the children were interacting with each other and with the provider in a positive way. One of the younger children was held by the provider while the older children were busy playing. A nice transition from playing with the work bench to a table top activity was made. The children sat in chairs at a table and used markers and crayons to draw. The provider reviewed the lesson of fire safety with the children and discussion on an upcoming visit by the local fire station is planned. All 6 children's records were reviewed. No medication needs at this time and no identified food allergies. Walk thru of the main program space, kitchen, bathroom and other rooms on the main level was made. Observation of the outdoor area and patio was completed. Previous violations found during the Initial Inspection were reviewed and corrective actions were verified. This inspection occurred between 10am and 11:30am. Thank you for your cooperation today. Please email me at: lisa.hudson@doe.virginia.gov with any questions.
Based on child record review, documentation that children have been adequately immunized were not obtained. Evidence: Documentation of immunizations for all 6 children in care were not obtained prior to beginning care and did not include a statement that they have been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Plan of Correction:
Going forward immunizations will be received before a child can begin care and they will include a signed/stamped signature from the physician that the child has been adequately immunized.
