Comments:
An unannounced renewal inspection was conducted today, 09/19/2023, from 12:40PM to 1:45PM. 2 children (2pts) were directly supervised by 1 caregiver. The physical space, required posted documents, sleeping areas, the Emergency Preparedness Plan, First Aid Supplies, training certificates, caregiver records and 4 children records were reviewed. The Caregiver is not MAT trained and will not be administering medications. Age appropriate activities and materials were available for the children in the classroom area. Children were observed working on individual activities. Areas of non-compliance are identified in the Violation Notice. Based on observation and interview with the Provider, required documents were not posted in a conspicuous place on the licensed premises. Evidence: The previous Inspection Summary was not posted.

Description:
Based on record review, 2 out of 4 children's records records did not have the parent's written acknowledgement of the receipt of information required to be given to parents before the child's first date of attendance. Evidence: Child A and Child B were missing documents for Parent Information, Limited Liability Insurance, Provisions of Emergency Preparedness and Response Plan and the policies for the administration of medications.
