Comments:
Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on February 27, 2024. There were 18 children ages 5 months to 3 years old present under the direct care of 4 staff. The Licensing Inspector reviewed 4 children?s records and 10 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review & discussion with the Center Director, the Center failed to obtain information regarding previous child day care and schools attended by the child.
EVIDENCE: 1. Record for Child #1 (date of enrollment 2/2023) did not information regarding previous child day care and schools attended obtained upon enrollment.
2. Record for Child #2 (date of enrollment 7/31/2023) did not information regarding previous child day care and schools attended obtained upon enrollment.
3. Record for Child #3 (date of enrollment 1/05/2024) did not information regarding previous child day care and schools attended obtained upon enrollment.
4. Record for Child #4 (date of enrollment unknown) did not information regarding previous child day care and schools attended obtained upon enrollment.
Plan of Correction:
Director will obtain previous schools/day care information for each enrolled child. Information will be kept in each child's record.
