Comments:
An unannounced monitoring inspection was conducted on 04/02/2024 in response to a self-reported incident. There was a total of 15 children and 5 staff members present during the inspection. The inspector reviewed video footage, interviewed 1 staff members and reviewed a total of 1 child record, 4 staff records and 1 injury record during the investigation. The inspector arrived at 11:40 am and departed at 1:05 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review, the facility failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.
Evidence: The record for Child 1 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (there were no addresses provided for either designated person).
Plan of Correction:
Will call the parent and get the missing information added to the record.

Description:
Based on document review, the facility failed to ensure that the written record of children?s injuries contain all of the elements as required by the standards.
Evidence: The injury record for Child 1 did not contain the following information: the time when parents were notified; and staff and parent signature or two staff signatures (there was only one staff members signature on the injury record).
Plan of Correction:
Will make sure that all required information is on the injury record.
