Comments:
An unannounced monitoring inspection, resulting from a self-reported incident, was conducted on 05/15/2024. The inspector reviewed video footage, interviewed 1 staff, and reviewed a total of 1 staff record and 1 child?s record. The inspector arrived at 11:15 am and departed at 2:40 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on video footage and interview with the staff, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection and guidance.
Evidence: Video footage from 05/02/2024 shows Child 1 (13 years old) sitting on a mat in the middle of the room. Staff 1 is seen walking towards Child 1 pointing their finger at the child. Child 1 is seen kicking at Staff 1, slapping Staff 1 in the face, and then throwing themselves back onto the mat. Staff 1 is seen holding Child 1?s hands to stop the hitting as Child 1 continues to kick at Staff 1. Staff 1 then stands up, places her foot underneath the mat and rolls Child 1 off of the mat onto the floor. Child 1 can been seen sitting themselves back up. Staff 1 did not provide the proper care, protection and guidance to Child 1.
Plan of Correction:
The staff member was terminated effective 5/3/24 after the incident occurred.
