Based on observation and staff interview, the center has not maintained a dated current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan Evidence: A allergy list was found posted in each classroom, this list did not contain children's sensitivities and was not dated.
Plan of Correction:
Food sensitivity will be included on the allergy list and the list will be dated.
Standard #:
8VAC20-780-60-A-8
Description:
Based on a review of medications, the center has not maintained a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The center has an allergy action plan for child #1 but it was completed by the parent and does not have the physicians instructions for the steps to be taken in the event of a suspected allergic reaction.
Plan of Correction:
The parent will have the physician complete the allergy action plan form and return to the center.
