Comments:
An announced initial inspection was conducted on 1/26/2023 from 10:40 am-1:30 pm. There were 50 children supervised by 13 direct care staff. The physical plant, 6 staff records, 5 children?s records, staff qualifications, background checks, policies and procedures and the emergency preparedness plan were reviewed. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Rooms are filled with supplies, equipment, and age appropriate activities for children. Areas of non-compliance are identified in the Violation Notice. Based on review of five children?s records, the center did not maintain complete children?s records.
Evidence: There was no documentation of the names, addresses, and phone numbers of two designated people to call in an emergency if a parent cannot be reached.
