A complaint investigation was conducted with the center on 2/27/2024. The complaint consisted of allegations received by the Northern Licensing Office regarding terms of the license. Interviews were conducted and observations were made throughout the physical plant. A preponderance of evidence supports the allegations regarding terms of the license, therefore, the complaint is deemed valid. Areas of non-compliance are identified in the Violation Notice.

Based on observation and staff interview, the center did not ensure that notice of the superintendent's intent to deny renewal of the license of the child day program was posted in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations.
Evidence: The notice of the superintendent's intent to deny renewal of the license of the child day program was not posted anywhere at the center.
