Comments:
An unannounced Subsidy Inspection was conducted on December 14, 2023. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the Vendor failed to ensure a person living in the home of a registered family day home provider has a sworn statement or affirmation when the person started living in the home.
EVIDENCE: Household Member #2 did not complete a sworn disclosure nor did they have a central registry background check.
