Technical Assistance:
Discussed access to classrooms from the outside and safety consideration of staff.
Discussed signatures on injury reports to demonstrate how and when parents notified.

Comments:
An unannounced monitoring inspection was conducted today from 11:35-12:45. There were 48 children directly supervised by 9 staff. The physical plant, 2 new staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, and eating lunch. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on review of child #1's file, the center did not obtain a physical record for child #1 within 30 days of the first day of attendance.
Evidence:
1. Child #1 started attending the center over two months ago.
2. The physical form was blank but attached to child #1's immunization records.

Plan of Correction: We will review all of the children's files to ensure each file contains a physical report.

Standard #: 8VAC20-780-60-A-7
Description: Based on review of children's records and staff interviews, actions to take in an emergency food allergy situation were not maintained in Child #1's and Child #3's files.
Evidence:
1. In each class, staff were asked about food or other allergies. Staff were aware of which children had food allergies and stated how they kept cross contamination of food from happening.
2. Child #1 and Child #3 had files that contained the food allergies. The forms did not include what actions should be taken in the event of an emergency situation.

Plan of Correction: We will have parents record what plan of action they want the center to take. We will also review children's files to ensure all listed allergies have a parents plan of action.

Standard #: 8VAC20-780-550-P
Description: Based on review of injury records, the center did not record future actions to prevent recurrence of each injury in the written record of children's injuries.
Evidence:
1. The center has a maintained written record of children's injuries. Each record included the date, time type of injury, and the names of staff present.
2. Each form reviewed did not include the steps staff would take to avoid future recurrence of the injury.

Plan of Correction: We will make corrections to the Injury Report so that plan to prevent recurrence is listed on the report.
