Comments:
A renewal inspection was initiated on 7/12/2023 at approximately 10:25am and concluded on 7/12/2023 at approximately 12:35pm. There were a total of 39 children with 12 staff members present. Children were observed engaged in group activities and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 child records and 7 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of staff information, and confirmed by the director, the center did not obtain an out of state criminal history check for all staff members.

Evidence: There was no out of state criminal history check for Staff #6.

Plan of Correction: The center will obtain the out of state criminal history check for Staff #7.

Standard #: 8VAC20-780-60-A
Description: Based on review of children's information, and confirmed by the director, records for each child did not contain the work phone number of each parent, or the phone number of two designated people to call in an emergency if a parent cannot be reached.

Evidence: There was not work phone numbers for both parents for Child #2, Child #3, and Child #4. There was not a 2nd emergency contact for Child #7.

Plan of Correction: The center will obtain the missing work phone numbers and emergency contact information for the children.
