Comments:
The Licensing Inspector conducted an unannounced inspection on 7/31/2023 in response to an anonymous complaint reported to the licensing office. Interviews were conducted with staff relating to the report of a staff member not disclosing a pending charge of a crime within the Commonwealth of Virginia when they signed their sworn statement. The information gathered during this investigation finds the complaint of a staff member not disclosing a pending charge to be valid. Based on interview with staff, the staff member did not disclose they were the subject of pending changes for an offence within the Commonwealth when they provided a sworn statement.
Evidence: Staff member was the subject of pending charges when they provided a sworn statement.
Plan of Correction:
The staff member was removed from employment at the center.
