Comments:
A monitoring inspection was conducted on November 27, 2023. The inspector was on site from approximately 11:50 am until 2:30 pm. The director was present during the inspection. There were 29 children present, ranging in ages from three to four years, with four staff. Children in Essex 1 had just returned from outdoor play and were washing hands. Children listened to a story and prepared for nap. Children in Essex 2 were in circle time talking about the concepts of more and less using counting cubes. Later children had a snack and prepared for dismissal. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Five children?s records were reviewed. Five staff records were reviewed at the administrative office on December 14, 2023. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of five staff records and interviews, the center did not obtain the satisfactory results of a national fingerprint-based criminal record search for one staff prior to employment. Evidence: The national fingerprints-based criminal record search in the record of Staff #5, employed on 09/02/2023, was dated 09/25/2023. During interview, a member of management confirmed Staff #5 started employment on 09/02/23 and the fingerprint-based criminal record search was obtained after.

Evidence: T he record of Child #1, enrolled 08/14/2023 did not contain documentation that the child received the immunizations required. The record contained a note from the child's physician indicating "4 year old vaccines deferred by family?.

Based on review of children's records and documentation, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record of child #4 confirmed a diagnosed food allergy. An allergy care plan was in the record however it was not complete nor was it signed by the physician. A note from the child?s physician was in the record and stated the child has an allergy to shellfish and if there were any questions to contact the office.? Both the note and the care plan did not include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Description:
Based on observation and interview, the center did not ensure the staff-to-child ratio for three-year-olds up to school age eligible were maintained whenever children were in care. Evidence: 1. The licensing inspector arrived and signed in at the elementary school office at 11:50 am. Staff #1 was observed in the office with a child for dismissal. Staff #1 returned to the Essex 1 classroom at approximately 11:58 am where staff #2 was alone supervising 15 children ages three years to four years. 2. Upon interview, Staff #2 stated when children have early dismissals or when staff need to retrieve lunch for the cafeteria, then one staff is in the classroom alone. 3. The required staff-to child ratio for children three years up to school age eligible is one staff to every ten children. Staff #2 was out of ratio in the Essex 1 classroom for approximately ten minutes
